Claiming of Business Rates

The NHS Dental Services are now enforcing the time limitation clause within the Statement of Financial Entitlements (SFE) with regard to claiming reimbursement for business rates. This clause has always been part of the 2013 SFE but not always enforced.

‘Time limitation for claiming payments

•11.13 Subject to paragraphs 11.14 and 11.15, contractors are only eligible for payments under this SFE if the payments are claimed within three months of the date on which they could first have fallen due. Therefore we would ask you to ensure any application for payment of business rates under the SFE is made to NHS Dental Services as soon as you receive your demand notice from the council. This year the NHS Mail programme made some changes to allow Pharmacists, Optometrists and Dentists (PODs) to register an account and access the service more easily. NHS England is encouraging dental and ophthalmic practices and pharmacies to apply for an account so that they are able to transfer patient identifiable information securely as well as having a dedicated email address that will improve communication with the Area Team.

The North Yorkshire and Humber Commissioning Support Unit (CSU) are providing this service on behalf of the Area Team; if your practice would like an account they will be able to assist you with the registration.

In order to be set up with an account the practice has to demonstrate they are Information Governance (IG) compliant by completing the IG toolkit supported by the Health and Social Care Information Centre (HSCIC). The CSU are currently in the process of checking which practices have completed the toolkit and will be contacting those practices over the next few weeks to assist with creating NHS net accounts. If you are currently in the process of completing the IG toolkit then the CSU will contact you once it’s completed.

Changes to NHS Patient Charges from April 1^st 2015

Band 1 increase from £18.50 to £18.80

Band 2 increases from £50.50 to £51.30

Band 3 increases from £219 to £222.50

Dental Prototypes

My single handed practice in East Hull has now been involved with the NHS dental pilot for eighteen months. Earl Howe announced early last year that the pilots will be replaced with dental prototypes this summer.

I feel that I am definitely a better dentist since joining the scheme. I have always tried to provide a high quality of dentistry, but having added a therapist to the team, I have realised that I can be treating periodontal disease more aggressively by carrying out scale and polishes more regularly. The vast majority of my patients were receiving a scale and polish at every check-up, but now, more are seen every three months. I have also realised that my communication with periodontal patients regarding the presence, diagnosis and treatment of their disease left room for improvement. This was a hard lesson to take, but as a result I now appreciate the ‘care pathway’ approach of patient management more and see its value.”

The Department of Health has published the next stage of the dental contract reform and is asking for dental practices to take part in the new dental prototypes. Existing pilot sites are being invited to apply, as are new practices who wish to take part in the scheme.

The prototype approach has three elements:

• a clinical approach focused on thorough assessment and prevention as well as treatment, and which supports a pathway approach to care

• Measurement and remuneration for quality of care

• Remuneration that supports continuing care and a focus on prevention as well as treatment/activity

This stage of reform will start in 2015/16. In this new prototype stage dental practices will test whole versions of a possible new system, rather than, as in the pilots, key elements needed to design a new system.

The DH says the prototypes will continue to test and refine the pathway approach used in the pilots with the same set of quality measures. As before the DQOF (Dental Quality Outcome Framework) will be looking for an improvement in patients’ oral health. However, they will not test the same approach to remuneration. The prototypes in contrast will be using a remuneration system that, while it may still need significant refinement, is intended to form the basis of a new system.

Drug interaction with Warfarin and Miconazole gel

In November 2014, 3 patients under the care of the Leeds Anticoagulant Service experienced INR’s > 8 due to the interaction between Daktarin (miconazole) oral gel and warfarin. One patient was admitted bleeding with an INR of 22.

This interaction is well documented and is flagged up on EMIS and System One. The combination SHOULD NOT be used together. The interaction is due to miconazole inhibiting the enzymes responsible for warfarin metabolism leading to high levels of warfarin and an increased risk of bleeding.

An alternative is Nystatin.”

With effect from December 2014, the Pradaxa Patient Alert Card will be provided in packs of Pradaxa capsules as a package insert. The card is intended to be carried by patients taking Pradaxa, and provides information to both patients and healthcare professionals regarding the safe use of Pradaxa.

The current Pradaxa Patient Alert Card is a nationally approved document, which is distributed to healthcare professionals by Boehringer Ingelheim in Pradaxa Educational Packs, via our representatives, or by contacting us directly. It is available to both healthcare staff and patients on request and through www.pradaxa.co.uk. This version of the card is due to be phased out and replaced by a new European approved document. The new card will contain the same content as the previous national card.

The first Pradaxa packs to include the new version of the patient alert card will commence distribution in December 2014 and by the end of April 2015 all Pradaxa packs marketed in the UK will include the alert card as an insert.
